Student Affairs Participation Award Application Deadline

Attention Med Students!
Please see the attached nomination form for the Student Affairs Participation Award.
The intention of the application is that the student be nominated by someone who can illustrate and verify the student’s active participation in, and contribution to, university and/or community activities. Students must be nominated by someone other than themselves. Nominations are most often made by faculty, but others in leadership positions affiliated with the university would be more than welcome to submit nominations as well.
The deadline date for this award is Friday, April 12, 2019.
